Measure B2 - Measure in support of non-self-sufficient elderly individuals with low assistance needs and persons with severe disabilities

OBJECTIVE OF THE MEASURE
Measure B2 is an instrument designed to support individuals with severe disabilities or conditions of non-self-sufficiency. It consists of interventions that provide assistance and support to the individual and their family, ensuring that the vulnerable person can remain in their own home and community environment.

ELIGIBILITY
The beneficiaries of this Measure are individuals of any age residing in Lombardy, who are being cared for at home and meet the following criteria: 

  • severe functional limitations that significantly impair their self-sufficiency and personal autonomy in daily living activities, social interactions, and community engagement; 
  • conditions of severity as certified by Law 104/1992 or those who receive an attendance allowance; 
  • with the following maximum ISEE values: sociosanitary up to a maximum of 25,000.00 euros and ordinary ISEE for minors up to a maximum of 40,000.00 euros.

TYPE AND CHARACTERISTICS OF THE ASSISTANCE
The support intended for beneficiaries of measure B2 is provided through:

  • indirect assistance (financial contributions);
  • direct assistance (provision of supplementary social interventions). 

The indirect assistance tools-financial contributions described below are alternatives:

  • monthly social voucher of €100.00 aimed at compensating the care provided by the sole family caregiver;
  • monthly social voucher up to a maximum amount of €800.00 in the case of regularly employed care personnel, even in the presence of a family caregiver; 
  • autonomy allowance: for individuals with severe disabilities who wish to implement their independent living project without the support of a family caregiver but with the help of a personally chosen and regularly employed personal assistant, and who meet the ministerial requirements of the Pro.Vi, a monthly social voucher up to a maximum of €800.00 annually may be granted. This voucher up to €800.00 can only be provided upon the existence of an independent living project agreed upon between the District and ASST, and can be awarded to individuals with a sociosanitary ISEE <= €30,000.00.

Direct assistance tools "integrative social interventions" are developed through the following actions: 

  1. relief services (respite interventions) that facilitate the substitution of the caregiving duties of the family caregiver:
    • home-based socio-assistance/protective services;
    • temporary admission to residential care facilities;
    • socio-educational/educational services for individuals with disabilities/non-self-sufficient individuals in socialising contexts (outside the home), also through the development of projects aimed at "respite"; 
  2. psychological support pathways (individual or group) aimed at the family caregiver; 
  3. training/instruction interventions to reinforce caregiving duties and the management of assistance for one's own family member with disabilities/non-self-sufficient. These interventions can also be provided in the presence of care personnel.

The interventions referred to in point 1 must be guaranteed by all territorial plans. The commencement of integrative social interventions is set for 1st June 2024.

The Territorial Areas are also assigned the responsibility for providing supplementary direct assistance interventions for beneficiaries of measure B1 with the presence of a family caregiver who ensures care work, for whom the need is identified during the integrated multidimensional assessment.
No specific application to the Territorial Areas is required from B1 beneficiaries.
For individuals eligible for Measure B1, the recognisable amount per individual project is €65.00/month up to a maximum of €455.00. These interventions may be utilised until 31.12.2024.

MODE OF ACCESS
The multi-professional multidimensional assessment, subsequent to the verification of access requirements, is conducted by the Territorial Areas in an integrated manner with the Local Health and Social Care Trust (ASST) where necessary in relation to the identified health conditions. The assessment process concludes with the drafting of the Individual Project.
The measure is allocated by the Territorial Areas following a public notice.
